Pine Tree Society is conducting a statewide Community Needs Assessment survey to capture a detailed picture of service needs among people living with a disability in Maine. Pine Tree Society will use the findings to better address challenges and barriers that prevent people living with disabilities from achieving a higher quality of life.

Within this 10-minute survey, there are specific questions for different types of community members, including:

Person with a disability

Caregiver of a child or children with disabilities (under 18)

Caregiver of an adult or adults with disabilities (over 18)

Provider of services for people with disabilities

Health care provider

School-based provider

Advocate

The survey is available here: surveymonkey.com/r/PTSCommunitySurvey

All responses will be kept confidential. This survey will close Friday, January 27, at 5 p.m. and the results will be shared this spring.
